We know that your time is valuable. Please use our interactive request forms for non-urgent requests. The information submitted through these forms will be securely routed to appropriate staff members.
Use our appointment request to schedule a well child, school or sports physical exam and/or ADHD recheck appointment. Call the office for other appointment needs.
Prescription refill requests should be made directly to your pharmacy. They will send us your refill request electronically through eScripts.
ADHD medication refills should be requested a week prior to your anticipated pickup date.
PE FORMS & IMMUNIZATION RECORDS
We cannot release any records to a thrid party without having on file a Release of Medical Records.
Prenatal Informational Meetings
Our Pediatricians offer evening "meet and greet" sessions at our office 2 times a month. These meetings are meant to provide soon-to-be parents an opportunity to meet our physicians, look at our facilities, and ask questions about the practice before selecting a pediatrician. The doctors cover common newborn topics as well as answer any questions you may have along the way.
